While you are here, please stop by and complete your child’s CYBHI consent form. Remember to bring your child’s medical/insurance information so we can make sure the form is complete.
CYBHI helps our school continue providing FREE mental health, emotional, and wellness support to students right here at school. There is no cost to families, and you will not receive a bill for these services.
If you provide all the information needed and complete the form, you will be entered into a raffle!

WED SEPT. 30th: Parent-Teacher Conference Day - NO SCHOOL FOR STUDENTS. All conferences will take place in the cafeteria between 9:00am to 12:00 and from 1:00 p.m. to 5:45 p.m. The cafeteria will be closed from 12:00 to 1:00 pm.

There is no cost to families, and you will not receive a bill for these services.
